Library Card Registration Form

Please fill out the form below if you wish to obtain a library card for the New Milford Public Library.
Once this form is e-mailed to us, we will make up a card for you and you may pick up your card within 5 days. Any questions, please call the Circulation Desk at (860) 355-1191 ext. 201

Please note: When you come in to pick up your card, please bring with you an item with your name and current address on it for identification. Also, for children's cards, a parent must be present when the child comes to pick up his/her library card.
